The Physician provides high quality patient centered primary care services utilizing a “care team” approach. In addition, the role is influential in implementing and advancing the Medical Home model practiced within the facility.

Duties and Responsibilities:

1. Delivers quality patient care in a respectful culturally sensitive manner that included acute and chronic care, preventative care, and consultations with patients, family and/or specialists

2. Diagnoses, treats and provides high quality patient centered care for medical conditions in children and adults following accepted medical standards of practice

3. Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions

4. Refers patients to specialists and to relevant patient care components as appropriate

5. Evaluates and revises patient care/treatment plan with patient and family as necessary to achieve improved health

6. Provides education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment

7. Provides collaborative oversight of mid-level providers, serving as a mentor and consultant

8. Provides accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential records

9. Participates actively in the advancement of FCP’s Medical Home model, including innovative ideas, function as leader of a Care Team, participation in care team clinical benchmarking, and meaningful use of the electronic health record

10. Collaborates with the Medical Director, Provider Team and Nursing Director to identify areas for system improvement, increased efficiency, communication and quality improvement activities

11. Develops relationships to provide for a broad spectrum of care with the various ancillary specialties available. (I.e. Rehab, Dental, Optical, Community Health, Behavioral Health, etc.)

12. Communicates within the department as well as across departments to ensure that information is shared for effective operations, and maintains strict confidentiality and adherence to HIPAA policies and procedures

Requirements (Education, Experience, Knowledge, Skills, Abilities):

* Doctrine of Medicine, with valid Wisconsin licensure.

* Current Board Certification, preferably in a primary care specialist such as family practice, internal medicine, or emergency medicine.

* Three to five (3-5) years’ experience practice experience in a primary care setting

* State of Wisconsin and Federal DEA Certification

* Knowledge of legal and ethical standards for the delivery of medical care

* Knowledge of community medical diagnostic and patient care services in area of medical expertise

* Knowledge of related accreditation and certification requirements

* Ability to supervise, advise, and train clinical professionals in area of expertise; work independently and in a team environment

* Must possess proof of applicable health department testing and vaccinations or testing/vaccination will be provided by FCP Employee Health

* Excellent Patient Relations and communication skills

* Ability to participate in clinic committees as requested
